All Items below have been verified by EMAIL accuracy (yes I still have my emails dating back to 2005) Things here are written in plain text no real details or this document would be 60 pages long.

In the following timeline I went from:

2005

  1. I attended Branford Hall Career Institute (Helped me jump start my Career)
  2. In the middle of the school year I was told by my Teacher (Jay El Hadad) to get my A+ certification so I went and did both my A+ and Net+ certifications within a month of each other prior to graduation.
  3. At the time of my internship a young lady named Jessica Arroyo put a word into the ear of the current Director of Information Technology at Fair Haven Community Health Center about me needing an internship. I interviewed and the same day I was working and learning.
  4. Later that year I was hired, and my career started.

2006

  1. I worked with the New Haven Board of Education to try and get some remote access to the main site from the schools. After many attempts I got Remote Desktop working into an old remote desktop virtual machine I built from a scrap desktop PC. This was a revolutionary task at the time since accessing the Patient Scheduling application from the schools was not done at the time and now it was possible!
  2. My first major disaster recovery attempt occurred this year where one of the raided drives on the server I inherited died and took all the data with it. I was playing with VMWARE that year making a clone of the fileserver 2 days prior to the failure. I used a software called Shadow Protect to take an image of the Virtual Machine and restore it to a physical machine. (I ask myself today why I didn’t just use the virtual machine moving forward. Instead I took the whole weekend to do a restore which had me spend the night there on the floor). We live and learn. Implemented shadow protect moving forward. Not bad for my first 6 months as an employee.

2007

  1. I worked tirelessly to maintain the 2 sites, 5 school-based health centers, and rollout a new site (WIC/Nurturing Families) next door alone for all 3 floors. I rolled out:
    1. Computers
    2. Printers
    3. Phones
    4. networking
    5. wifi
    6. and worked with AT&T to rollout fax lines and a T1 connection
    7. and worked with the contractors to understand how many cat 5 lines to run
    8. and worked with the state to get the network up and running for WIC.

2008

  1. I worked with Katrina Clark and EMT to plan on what our new building (Finance/Billing office) would look like. They had me work with the architects to plan how many network drops would be needed in the space.
  2. We extended the front of the building and added one more level (phase 1A “Now Semeon Tsalbins” and basement space “Now IT”)
  3. Later that year I brought on an intern from Branford Hall Career institute which then later became my employee finally giving me my number 2.
  4. We also expanded our Mitel system from holding 256 devices to 512 and added fiber to the main site for PRI communications.

 

2009

  1. My assistant and I went on to rollout the hardware in January to the new site (Finance/Billing):
    1. Computers
    2. Printers
    3. phones
    4. wifi
    5. networking
    6. and worked with AT&T to rollout fax lines and a T1 connection
  2. EMT implemented another construction change at the main site and we ran point on all IT related items. We had to implement the changes to:
      1. Provide access to the additional exam rooms
      2. PODs
      3. New Large Conference Room
      4. Decommission old conference room to segregate the space for additional offices
      5. New Bathrooms were implemented
      6. A dumb waiter to allow charts to move from the basement to the first floor
      7. A new Call Center space in the basement
      8. 2 additional rooms on the second floor in what is now the behavioral health wing
      9. New rear entrance and stairs that go up to the second floor and down to the basement
      10. We provided TV’s and computers to all the conference rooms
      11. Later that year we rolled out Spiceworks to assist with Ticket/Request Tracking and an intranet calendar for nurses to keep track of their schedule.
      12. The EHR wars began. This is when we began to look at the cost and options of servers and hosting options

2010

  1. In June of 2010 we signed the contract with Sage Intergy to utilize the Practice Management piece of the system to get off of our antiquated PCN dos system we had been using for years.
  2. Program a new phone tree and have user record the prompts

2011

  1. Rollout of this system was hard since we had to install it on every system in a specific way and there was no automation at the time to 200+ devices and create new accounts for everyone.
  2. Later that year we did an EPIC Technical Assessment with Yale
  3. Changed the firewall appliance to Cyberoam since a more secure network was in need

2012

  1. An EPIC implementation planning meeting was held with Yale
  2. Replaced all computers at the main site in preparation Yale EPIC installation received computers in 3 waves
  3. Go live was between Oct and Nov for all sites to go live in a staggered fashion.

2013

  1. We went live with our new email provider safety send
  2. We also went on a search for a new Electronic Dental Record and chose Dentrix and Dexis for managing.
  3. Received the Super Hero award from CHCACT

2014

  1. We transfer all email accounts to Office 365 which was a huge migration successfully
  2. Rolled out Active Fax Implementation site wide
  3. Dentrix Hardware/Software implementation
  4. The same year we rolled out a new dental truck and gave remote access to the EDR with a hotspot and a few laptops.
  5. Create yearly budget

2015

  1. We RFP for a new VOIP phone system, network, OPTEMAN’s for current T1 connected sites
  2. We Choose Total Communications after the bid war was complete
  3. Rolled out new wireless infrastructure to 5 sites
  4. Dexis Hardware/Software implementation
  5. Replace old backup methods with new hardware
  6. Rolled out replacement server for fileserver failure
  7. Rollout Finesse Call Center application and new Phone tree on the new phone system
  8. We Open a new site for Women’s health and other healthcare departments and rollout

    8. and worked with AT&T to rollout fax lines and OPTEMAN connection

2016

  1. Plan with contractors and EMT the opening of a new clinic in East Haven
  2. Expanded Yale EPIC services to all sites listed
  3. My assistant and I created a mini network with a hotspot, laptop, small router, and an wireless access point to provide services while the fiber issues were fixed with Frontier for 2 months
  4. Create Yearly Budget

2017

  1. We worked with Yale to implement MIPACS and wisdom
  2. Expanded Yale EPIC services to all sites listed
  3. We also opened 2 new School Based Health Centers in East Haven
  4. Implemented new credit card vendor Worldpay

2018

  1. Implemented Go to Meeting and safety huddle communications
  2. Enhance Staff Meetings by purchasing equipment

2019

      1. Created all usernames for all new employees
      2. Had them all login prior to starting their shifts
      3. I oriented them personally in order to give them that personal touch.

      2. We worked with comcast to get internet there as fast as possible since hotspots were for temporary use
        1. Once installed we programmed a meraki switch and unifi hotspot to create a network onsite
    27. My team and I imaged and configured 22 laptops to replace the laptops at 50 Sargent Drive.
      1. Once configured my team and I rolled out the laptops 1 by 1 on site while the site was operational
        1. This task consisted of swapping out names of computers and programming EPIC access
    28. We moved on to rolling out the call center at our 2nd floor East Haven Office
      1. Imaging 22 computers after they were ordered and delivered
      2. Rolling them out to the site
      3. Placing Ethernet tips on over 40 cables (80+ Tips)
      4. Configuring network switches and wireless for the site
      5. Increasing the site to site fiber connectivity to handle the traffic of calls going to this site.
